Thanks for your response. With more logging of options selected by me along the way, I got back to the ending. Your input helped me to understand why.
However, . . .
I'm still in search of the third ending where Brad is the odd man out -- I'll certainly keep working on it. Any suggestions?
Gonna see if Kiss Pass HJ does the trick . . .

Without completely giving it away... After hours of testing different things, some of the key steps that make or break 3 pillows as far as I know.
(club)zipped/unzipped, (taxi)hand in/away from azumi's crotch, (apartment)It did seem that there was only one line of responses that led to three pillows, which was pretty easy to find once you were able to get to that point, it's the getting there that can be tricky
^ this isn't a walkthrough, but I have compiled one for people that have continued to struggle and can't seem to find it. I won't post it in here in a spoiler, but if it's allowed, I can send it in a message. Though it's so much more satisfying figuring it out.

Glad you are enjoying the game.
That particular ending requires getting both girls "interested." It is pretty simple to get one or the other but tricky to do both.
There are key steps on this path in the Strip Club, the Taxi and Home.
In the club, make sure you get through the private dance with pants unzipped. You do that when Natalie comes in and calms Azumi down. Do not get Candy so worked up that she has sex with Brad as she will zip him back up afterward. When Azumi comes back in, concentrate on pushing her points up. She is interested if Brad shows her that he can resist/detect her influence.
In the Taxi, don't be too grabby too soon. Talk to the girls first. When they notice Brad is unzipped, move to zip back up then go all in. Let Azumi move things along at first. You have to reach the point where Natalie gives Brad a very "special kiss" to reach the same sort of thing later in the apartment. This "kiss" is an achievement by itself. You won't get one later if you don't get it here.
Just FYI, my scores for the girls when arriving in the front hallway of Brad's apartment on this part were N=26 and A=29 with Reputation points at 11. This is just a ballpark. The scores could be a few higher or lower and still work.
At the apartment, keep earning points with both girls. Don't fall for Azumi's tricks - she will try more than once. At breakfast, be interested, not creepy. Take the "You still love each other" path. If you have enough points, they will go to the bathroom. Wait. Again, if points are high enough, they will dance for Brad. From there, keep going for the long-term choices. Hold off at every opportunity.

Hmmm . . .
Feeling a little sheepish right now -- what I call the Brad Odd Man Out ending (pants off with Natalie and Azumi - on same path as WhoWon and AngryNatalie) actually was actually pretty simple. As is sometimes the case, I was looking for a more complicated solution, which is why I couldn't get there.

Hi --
I've tried to very carefully follow the able advice in this forum, but still can't get Activity: Azumi Wins or Pants - Natalie Cum 2. I understand that both are on the KP path and can easily get there, have met points thresholds along the way and have carefully set commonly discussed mood triggers.
No luck tho ... Starting to wonder if my achievements file is corrupt.
Any ideas welcome ...

Actually, that is not where you seem to think it is. (Big Spoilers)
At the point where the "fight" occurs - Natalie tries to pull Azumi off of Brad and Azumi resists, hanging onto Brad - there is an internal status check. If the Cock Kiss happened in the cab, the game continues on to Natalie licking and the "3-pillow" ending. If the Cock Kiss did not happen in the cab, the game will route to Natalie Joins. There are two endings to Natalie Joins - Azumi accidentally slips onto Brad and Natale gets mad or Natalie cums and Azumi Wins.
I'll have to look for your other one.

I've tried that route literally hundreds of times and I always end up with the "friendly" Nat on her knees ending. Thus far, the only way I've been able to get to the Angry Natalie ending is on the KP Cup route. And, given that I've gotten there using the KP Cup route, by definition it precludes the SK result.
When I start using the SK route, I can tell in the second screen of the dance "game" that it's not right because I don't get the "would like Natalie to finish what she started" language.
Ugghhh... What am I missing ?!?!?!?! i'm quite sure it's something obvious and I'm blind to it from so many attempts . . .

There are several things that impact this one.
If you got the Natalie Pant Cum Full achievement, you did everything right for Pants Nat Cum 2 except you had too many points. The decision as to how Brad responds when Natalie asks if he thinks Azumi is hot will set Azumi's mood, which is key for a later check. Then, at the point where Brad has Nat on his lap and is "taking liberties," there is a point check. If Nat is 25 or greater, you go on to Nat Pant Cum Full. If she is less than 25, you route to Pants Nat Cum 2. Be careful and don't blow off too many points or you will fail an earlier check. At that point, 24 would be perfect.

You seem a little distracted. Azumi Wins is "almost the same" as
Natalie Angry. When you choose to penetrate Azumi you get Natalie Angry. If you choose to cum just BEFORE penetrating Azumi you get Azumi Wins.

I've gotten the result for Azumi Wins as you describe it -- it's just not reflected in my Achievements. Hence, my wonder about whether my Achievements file is corrupted. I figured it out days ago by generating exactly the result you describe -- no change tho to my Achievements -- still has a blank check-box next to Activity: Azumi Wins.

Did you continue all the way to the end of the scene? Some of those achievements won't be credited until you finish the scene and start Thursday Morning at the coffee run. If you finished it as listed and still don't see the achievement, it may be a bug.

Yes, I continue to the Thursday Coffee Run each time I complete a path . . . I don't complete the Coffee run, but I do go to the point where I'm crossing the street. I'll try progressing a bit further, but all the other achievements have posted by going to that point, so I'm pretty sure I'm going far enough . . .
Thanks again . . . I'll let you know what happens once I do a clean reinstall with a fresh Achievements file.

It's not a bug: it's part of a tidying up process that is pretty standard practice for an episodic game (or any well-structured game for that matter).

My suggestion is to follow the path you know works to the decision point for the "3-pillow ending." At that point, make a save file and proceed. If it doesn't fire, go back and try again but create a Bug Report after you finish that path again. The "longer" the bug report the better. One that shows the exact path you took from the start of the episode to the ending that didn't return an achievement as expected will help Wolf or Kexter figure out what went wrong or - more likely - where you went off track. You don't have to actually file the report if it isn't needed but having it makes it a lot easier to track down problems. Back when I was just a player, I sent one in and Wolf was able to get back to me with the exact point I missed something.
